    The Importance of CCF

    The Chain of Custody Form (CCF) is a legally binding document that traces the path of a specimen from the donor to the laboratory, and finally to the Medical Review Officer (MRO). It guarantees that the sample has not been adulterated or swapped.

    Donor Identification: The donor must provide valid photo ID before any collection begins.

    Secured Environment: The collection area is secured to prevent access to water or chemicals that could adulterate the specimen.

    Tamper-Evident Seals: Seals are placed over the specimen bottles in the donor's presence, and the donor initials them.

    Collection Site Protocols

    Collectors must strictly adhere to the following steps:

    • Ask the donor to empty their pockets and wash their hands before providing a sample.
    • Check the specimen's temperature and volume immediately after collection.
    • Verify that the information on the bottles perfectly matches the accompanying CCF.

    Laboratory Processing

    Accessioning

    Upon arrival at the laboratory, the package is inspected for signs of tampering. Seals are verified to be intact.

    Testing Phase

    The specimen is tracked internally through every stage of testing, ensuring a continuous documented chain.

    MRO Review

    Results are securely transmitted to the Medical Review Officer for final verification and reporting.
